Mission Home Health of Ontario offers a comprehensive suite of home health services, all provided by a multidisciplinary team of licensed and highly trained professionals. These services are designed to meet a wide range of medical and rehabilitative needs, all under a personalized plan of care developed in consultation with your own physician. The care is not one-size-fits-all; instead, it is tailored to the specific diagnosis and medical requirements of each patient.
The core services provided include:
- Skilled Nursing Care: Provided by registered nurses who can manage, observe, and evaluate a patient's overall care. This includes direct care, managing complex medical conditions, medication administration, specialty wound and skin care, and patient/family education on disease management and recovery.
- Physical Therapy (PT): Essential for patients recovering from surgery, injury, or illness, or those dealing with physical decline. Physical therapists work to restore mobility, strength, balance, and coordination through mechanical means like exercise and massage, helping patients return to their usual activities.
- Occupational Therapy (OT): Focuses on helping patients regain the skills necessary for daily living activities, such as bathing, dressing, preparing meals, and housekeeping. OT is crucial for enhancing or regaining independence after a physical, developmental, or cognitive impairment.
- Speech Therapy: Services provided to assist with problems involving speech, language, swallowing (dysphagia), voice disorders, and cognitive communication disorders.
- Behavioral Therapy: Counseling and support for social and emotional concerns related to an illness, which may include helping patients and families find necessary community resources.
- Nutrition Counseling / Dietary Consulting: Professional guidance to help patients manage health conditions through diet, essential for conditions like diabetes management or post-surgical recovery.
- Home Health Aides: Assistants who provide help with daily living activities (ADLs), such as getting up, bathing, dressing, and making a meal, under the supervision of a registered nurse.
- Medical Social Services (MSS): Providing support for social and emotional concerns related to illness and connecting patients with community resources.
- Durable Medical Equipment (DME) & Supplies: Coordination of necessary medical supplies and equipment to support care at home.

Whether you are a California healthcare provider seeking to make a referral or a family member looking into options for a loved one, these channels are the most direct way to begin the conversation about personalized in-home care.
Choosing a home health care provider requires weighing professional service quality against customer support responsiveness. Mission Home Health of Ontario presents a strong case by offering comprehensive, medically supervised services within the home environment, a key factor for maximizing comfort and positive patient outcomes. The array of specialized therapies—including Physical, Occupational, Speech, and Behavioral—combined with Skilled Nursing Care and assistance from Home Health Aides, means they can capably handle a wide spectrum of post-acute and chronic care needs. This specialization in multiple areas of in-home therapy makes them a robust choice for complex rehabilitation.
Patient testimonials often highlight the professionalism and knowledge of the nurses and therapists, noting that the clinical care itself is "outstanding" and that staff are "most helpful at making sure things ran smoothly." The commitment to assigning a consistent therapist, rather than constantly switching personnel, is a key benefit noted by families, as it fosters continuity and trust in the care relationship. Furthermore, the agency’s capacity for 24/7 patient admission and around-the-clock availability for support are crucial features that offer security to families navigating stressful medical situations.
However, potential users should also be aware of customer feedback regarding administrative responsiveness. While the clinical staff and direct care are consistently praised, some past reviews have pointed out challenges with customer service, specifically difficulty reaching a live person by phone and delays in scheduled visits. This suggests that while the clinical expertise of the nurses and therapists is top-tier, families dealing with immediate or time-sensitive needs may need to be proactive in follow-up, especially regarding scheduling and administrative communication. Terry, mentioned in one review, appears to be a notable staff member who is helpful in smoothing out operations, indicating that positive experiences often hinge on specific staff members. Ultimately, for those in the Ontario, CA area prioritizing high-quality, professional in-home medical and rehabilitative therapy, Mission Home Health of Ontario offers a clinically strong option that is dedicated to supporting the patient's journey toward health and independence at home.

★ 5★ 4★ 3★ 2★ 1My mother was assigned to mission home health, when you contact this company no one picks up the phone, but rather you are directed to leave a message where no one calls you back, A nurse was previously scheduled to show up at my mothers residence between 8:00 and 8:30 to place an iv, after a no show it was not until almost 1:00 pm that someone finally showed up and this after leaving multiple messages. Apparently Mission home health believes that a message machine is adequate rather than a live person. So much for 24 hour service.. Patints and their families that are dealing with various stressful situations should have the ability to speak with a person rather than an answering machine. and the courtesy of a return telephone call My complaint is not with the nurses but rather mission home health’s customer service
July 31 · William Tiltonhealth care i received from the ontario office was outstanding. the nurses were very professional and knowledgeable..Terry was most helpful at making sure things ran smoothly. I looked forward to my visits.. I highly recommend Mission home health care.. Ed Johnson
June 17 · Edward JohnsonI have been sick for the past nine years. I am so happy that mission has been there to help me. I have not had any problems or issues. I have had to have nurses, physically therapy and otc. Luckyly I had the same people most of the time.
June 11 · linda whiteIf I could give negative stars, I would. Unfortunately, Mission was the only option for home health when my husband was discharged from the hospital seven months ago. The nurses changed weekly with zero communication until the day before arrival on what time they would be coming. Some nurses were great, most were rough and didn't spend more than 10 minutes changing wound dressing and taking vitals. It was very transactional and not personable. When my husband returned to work, Mission simply canceled us and washed their hands of his care. Meanwhile, he still had an active drain that required dressing changes and saline flushes several times a day- all of which we could handle on our own, but Mission was supposed to ease that transition and ensure that we continue to get the supplies that we need. The hospital and insurance have repeatedly told us that Mission Home Health is responsible for ensuring we have the supplies needed to continue care. However, NO ONE at Mission did that. We talked to "managers" and no solutions were provided and no assistance given. They left us high and dry. Thankfully, my husband no longer requires assistance or supplies so we don't have to deal with Mission again, but, God forbid we ever need home health in the future. I would never recommend Mission. Never.
May 10 · Cassandra MortonTeri was my home health nurse and she was fantastic. She took care of me and asked me a lot of health questions and was knowledgeable and wise about giving advice related to my treatment. I would recommend her as a home health nurse because she is very attentive and communicates well with my regards to my needs. A true caring nurse!
