Grace Home Health Care in Long Beach, California, is one such provider, offering a comprehensive range of medical and rehabilitative services designed to allow individuals to recover or manage chronic conditions in the familiar and comfortable environment of their own home. As a licensed, Medicare-certified, and CHAP-accredited agency, Grace Home Health Care is committed to delivering professional care tailored to each patient's specific needs and physician's plan.
The core mission of home health care is to provide integrated and cost-effective services to individuals and their families, regardless of age, focusing on the patient's physical, psychosocial, and spiritual well-being. This approach ensures that patients in the greater Long Beach area of Southern California can receive the necessary support to maintain their independence and enhance their quality of life outside of a hospital or skilled nursing facility.
The agency’s commitment is often summarized by its mantra, “Service from the heart.” They work closely with patients, their families, and their personal physicians to develop and implement an appropriate and effective plan of care. This collaborative model is fundamental to successful home-based recovery and long-term condition management.
Potential clients and referring physicians seeking home health services need to understand the specifics of the services offered, the agency's credentials, and how to access their support. This detailed overview is intended to serve as a resource for those in the Long Beach community and surrounding California regions who are exploring their in-home care options.
Grace Home Health Care was established in 1995 and has maintained its status as a licensed and certified provider in the state of California, accepting both Medicare and Medi-Cal, which is a significant factor for many families managing healthcare costs.
Grace Home Health Care is conveniently situated in the city of Long Beach, a major hub in Los Angeles County, California. Its central office acts as the operational center for coordinating care for patients throughout its service area. The agency’s physical location is essential for administrative and staff coordination, though the majority of patient care is delivered in the client's residence.
The office is located at a readily accessible address, which aids in logistics for staff, patient consultations, and administrative matters. For local residents, knowing the physical location can provide peace of mind and simplify any necessary in-person communication.
The main office address is:
1739 Termino Ave, Long Beach, CA 90804, USA
For clients or visitors who may need to visit the office, a key feature that enhances accessibility is the provision of a:
- Wheelchair accessible parking lot.
This attention to physical accessibility demonstrates a fundamental understanding of the needs of the community the agency serves, which often includes individuals with mobility challenges. The location within Long Beach positions the agency well to serve a wide demographic across the city and neighboring communities in Southern California.
Grace Home Health Care offers a robust suite of services, administered by a team of well-trained and licensed professionals, including skilled nurses, therapists, and home health aides. These services are often initiated following a hospital stay, a change in medical condition, or to manage a long-term illness, and always require a physician’s order for Medicare or insurance coverage.
The extensive list of services is designed to cover both complex medical needs and essential daily living assistance, promoting comprehensive recovery and self-reliance in the home setting. These offerings include:
- Skilled Nursing Services: Professional nursing care for complex medical needs, including wound care, ostomy care, post-surgical care, PICC line/Central line and IV access management, Intravenous Therapy (such as TPN, antibiotics, and pain management), glucose monitoring, insulin therapy, Foley Catheter Care, Tracheostomy Tube Care, Oxygen Therapy and other breathing machines (like Ventilators), Apnea Monitoring, and care of chronic medical conditions. Nurses also provide personalized teaching to patients and caregivers.
- Physical Therapy (PT): Unique and individualized treatment plans focused on regaining or maintaining mobility and restoring function. Services include gait training, training on the use of Durable Medical Equipment (DME) such as canes, walkers, and wheelchairs, energy conservation techniques, and developing fitness-oriented and wellness-oriented programs.
- Occupational Therapy (OT): Customized care plans aimed at improving health and self-reliance in performing daily living activities. Occupational therapists guide patients and/or caregivers in the use of therapeutic, creative, and self-care activities to improve self-care functions.
- Speech Therapy (ST): Interventions to restore language and communication skills, regain impaired communication abilities, or explore new ways to communicate one's feelings, tailored to the patient's needs. Services may also address swallowing and cognitive issues.
- Home Health Aide Services: Essential assistance with daily living activities (ADLs) such as getting up, bathing, getting dressed, personal care, meal preparation, and medication supervision. Aides provide necessary support to help clients maintain dignity and independence.
- Medical Social Services: Professional support for patients and families facing health-related challenges, including counseling, resource coordination, and assistance with navigating the healthcare system to ensure access to necessary community and medical resources.
- Dietician Services / Nutritional Guidance: Professional advice and support on dietary needs to improve patient well-being, manage chronic conditions, and support the recovery process.
Beyond the core services, Grace Home Health Care possesses several key features that highlight its structure and commitment to care within the California health system. These credentials are vital considerations for patients and families:

Choosing a home health care provider is a critical decision for patients and their families in California. Grace Home Health Care presents itself as a well-established and comprehensive option for several reasons that are worth considering.
The primary advantage is the breadth of its medical and therapeutic services. The extensive skilled nursing services, ranging from complex IV therapy and ventilator care to chronic disease management and personalized teaching, indicate a capacity to handle a wide spectrum of patient needs in a home setting. This level of skilled care, coupled with robust rehabilitative therapy services—Physical, Occupational, and Speech Therapy—creates an integrated care model designed for holistic recovery and maximum independence. Patients are able to receive rehabilitation tailored to their specific living environment, which often yields better long-term functional results.
Furthermore, the official credentials are a major deciding factor. As a Medicare and Medi-Cal certified agency with CHAP accreditation, the agency has met rigorous federal and state standards for quality, safety, and operational excellence. This certification is essential for Californian patients relying on government-funded healthcare programs for coverage, simplifying the payment process.
The presence of Home Health Aide Services, Medical Social Services, and Dietician Services ensures that the care provided goes beyond just medical tasks. It addresses the emotional, social, and nutritional needs of the patient, which are crucial components of overall well-being and recovery. This integrated and multidisciplinary approach ensures that the patient’s health is viewed from a holistic perspective.
For potential referring physicians and health systems in Southern California, an agency’s ability to communicate clearly and efficiently is paramount for patient continuity of care. While some third-party feedback has indicated challenges in the referral process, the agency's long-standing presence since 1995 and its commitment to a 24-hour answering service suggest established protocols for managing patient needs, even after hours. When making a choice, it is always advisable to engage in direct dialogue with the agency's intake coordinator to confirm the seamless transfer of patient information and care coordination.
In summary, Grace Home Health Care in Long Beach, CA, stands out as a strong local choice, providing diverse, certified, and accredited home health care that supports patient independence and recovery within the comfort and familiarity of their own home.

Grace Home Health Care Reviews
Absolutely terrible service for referring physicians. After 1 month of successfully faxing a referral to the CORRECT fax number, my patient reaches out to me to tell me he's never been contacted. I called the company to follow up, and the person on the phone BELEAGUERED me for not following up to make sure that they received it and that the patient would be contacted. I am sorry, but PROFESSIONAL companies apologize for their mistakes, rather than incessantly telling the customer that they didn't do their job correctly. Coming from a renowned institution with many disabled patients from the Long Beach area, you would think they would be more respectful. I am NOT a case manager, in fact, I am a clinician and my job is not to follow up on every single order I send out. This is the first home health company that has not been able to follow through, and with the way I was treated, I won't be sending referrals here anymore.
