Professional Nurse Partners offers a suite of integrated services designed to support the holistic needs of older adults and those with disabilities, allowing them to maintain dignified and independent lifestyles within their preferred living environment—be it their own home, an assisted living facility, or another residential setting. The services are rooted in a comprehensive, clinical approach:
- Comprehensive Geriatric Care Management: A holistic approach where a Registered Nurse Care Manager acts as a guide and advocate, coordinating all aspects of the client's care, including medical, emotional, social, and functional well-being. This often involves developing a dynamic care plan following a thorough assessment.
- In-Home Care (Care Partner Program): Providing direct support to meet the needs of daily living for both the patient and their family. This program focuses on placing trained and caring personnel who are selectively matched to the client's unique requirements, helping to maintain safety and comfort at home.
- Medication Management: Specialized RN oversight to monitor medications, ensure prescribed regimens are followed correctly, and reconcile prescriptions to prevent errors, which is a critical service for clients with complex health conditions.
- Wellness Programs: Proactive programs focusing on health education, disease management, and promoting self-management of medical conditions. Registered Nurse Care Managers and wellness coaches work to foster overall well-being and disease prevention.
- Chronic Disease Management Support: Developing specialized care plans for individuals and their loved ones dealing with chronic or acute medical conditions, recognizing the challenges and overwhelm this can often bring.
- Adults with Disabilities and Special Care Needs: Providing long-term care management tailored to the specific needs of adults requiring specialized support beyond typical elder care.

The "Professional Nurse Partners" distinction lies in several key features that enhance the quality and reliability of care for California families:
- RN-Owned and Managed: The organization is professional nurse-owned, meaning that all programs and services are directed and administered with a high standard of clinical expertise and knowledge of healthcare policies and medical ethics.
- Clinical Specialization: Services have a specialized clinical focus, utilizing Registered Nurse Care Managers to educate patients and oversee health and disease management, which is a significant value add over non-clinical home care providers.
- Holistic, Client-Centered Approach: Care considers the individual’s emotional, social, and physical well-being, focusing on the client's ability to perform daily tasks and activities of living, thereby protecting their overall quality of life.
- Crisis Avoidance Focus: A core benefit of their Care Management process is initiating appropriate care and resources to prevent crises, avoid unnecessary hospitalizations, and ensure appropriate placements, ultimately saving money and reducing family burden.
- Strong Professional Endorsements: Highly recommended by local Elder Law Attorneys and Estate Planning professionals in the Orange County area, demonstrating a strong, trusted reputation within the professional community.
- Commitment to Dignity and Independence: The team is passionately dedicated to helping clients lead dignified, independent lifestyles in the comfort and safety of their homes for as long as possible.

Professional Nurse Partners Reviews
I am an Estate Planning/Elder Law Attorney and I am often asked who I would recommend to provide in home nursing care. I recommend Professional Nurse Partners with confidence that they will provide excellent compassionate care that is appropriate for the client's health and best interests.I have referred difficult clients with good results. And I have worked with clients who have received excellent long term care from P.N.P., and I am extremely satisfied with services provided and the results achieved.
December 15 · Gerard O'BrienProfessional Nurse Partners provides excellent care to their elderly clients. The staff is friendly, professional, and easy to work with. Everyone is very committed to the clients and care for them as if they are their own family. It is their goal to improve their well-being and quality of life. I would highly recommend their services for anyone who is looking for elder care.
December 14 · Katie LeonardThe staff at PNP and My Nurse Wellness Coach are wonderful. I worked with Debbie on a 10 day blood sugar program. It was the best thing I could of done. It showed me how eating healthier mad me feel better. Debbie and her staff are the best. They are with you all the way and provide the most outstanding support. Thank you Debbie for showing me a better way of living.
August 10 · Michele WhiteProfessional Nurse Partners is a wonderful agency that provides care for the elderly. As we age we still need the quality of life we alway had. They are professional, caring and compassionate. Extremely impressed with their services. I highly recommend them.
December 31 · Ana RhoadsOur family had a horrible experience with Professional Nurse Partners (PNP). I am writing this review because I want to help others avoid the stress, frustration and very poor quality of service my parents and family experienced. What went wrong? PNP does a good job with their up front sell. They present all the great things they say they will do but they fail miserably on what counts most. Here are some of the places where they failed: 1) Staffing challenges: promised min. Per day of 4 hrs. Home care. Reality was after signing and paying high amount for their required "initial assessment", which took months for them to complete, they couldn't find staff (assuming they don't pay enough) to work 4 hrs so shifted to saying 8 hr. was needed. Note, when we switched to another provider, they have had no problem proving excellent home care support for 4 hr. shifts. 2) Poor Follow-up: PNP constantly said they would do different things (such as set up a medicine dispenser) but never followed up on what they said they would do. Constantly had to chase them, they always had crazy excuses, still didn't do what they said they would do. 3) Inept at working with Long Term Care Insurance (LTC): PNP said they were experts at working with LTC providers and invoicing. Reality was, they did such a horrible job with invoicing, the LTC had to keep asking them to resubmit, PNP charged my parents over $1000 they billed (like attorney's) for time they charged to fix their own billing errors and yet they never fixed this and they walked away without resolving. 4) Poor Communications: PNP wouldn't return calls or emails/ texts for days and always had some poor excuse such as phone issues, traveling and out of range or the care manager would go on vacation with no notification or provided back up. While there were some things PNP did fairly well and perhaps others might have differing opinions, our family's experience was very negative. So disappointing.
